From “infant colic” to infant distress syndrome: broadening the clinical perspective

For many years, the term “infant colic” was used almost automatically to describe any discomfort in the baby associated with crying, irritability, or difficulty calming down. However, international organizations have begun to modify this nomenclature and are increasingly using the term “infant distress syndrome.”

And this change is much deeper than a matter of name.

We love this broadening of the perspective, because it stops reducing the problem solely to the digestive system or to the visible symptom, and begins to consider that, behind a baby with distress, multiple factors may coexist.

That said, the word “syndrome” should not become a big drawer where everything is thrown in without clinical reasoning. Rather, it invites us to observe in greater depth what is happening in each baby and in each family.

Because in clinical practice, we constantly find factors that interact with one another:

musculoskeletal factors,
respiratory factors,
alterations in the baby’s sleep and rhythm,
the way the baby feeds,
sensory regulation,
the environment,
family stressors,
the caregiver-baby relationship,
parenting strategies,
and many other elements that participate in how that child manages — or does not manage — to organize themselves.

From a PIMT perspective, this makes a great deal of sense.

Pediatric physiotherapy should not be limited solely to “calming” a symptom or applying isolated techniques. The real clinical challenge lies in understanding which factors are sustaining that state of distress and how they impact the baby’s and family’s participation and everyday life.

Sometimes we find babies with difficulties in bodily regulation, sustained postural tensions, respiratory alterations, feeding difficulties, or highly demanding environments that end up overloading their adaptation strategies.

And many times, when we manage to integrate that information into clinical reasoning, therapeutic decisions change completely.

The way we support the family also changes.

Because when families understand that we are not looking for a single cause or one magical solution, but rather understanding the baby globally, guilt decreases and a more human, functional, and respectful intervention emerges.

In pediatrics, broadening the perspective does not mean making things unnecessarily complex. It means understanding better.
