Clinical key points to understand why the vestibular system is structural in pediatric physiotherapy

In pediatric physiotherapy, we do not treat muscles, reflexes, or motor milestones in isolation.
We treat developing systems.
One of the most determining and yet often invisible systems is the vestibular system.
Not because we “control balance,” but because it organizes the relationship between movement, posture, and space.
The vestibular system acts as an internal reference system that allows the brain to answer three basic questions:
Where is my body?
Where am I moving to?
Which part of my body needs to be activated or stabilized?

Clinical key points we observe in daily practice
When the vestibular system is not well integrated, signs often appear that are commonly misinterpreted as partial issues:

  • Fluctuating or unstable tone
  • Difficulty maintaining postures without excessive visual fixation
  • Stiffness in postural changes
  • Insecurity on unstable surfaces
  • Avoidance of rotational movement or, on the contrary, disorganized searching
  • Early postural fatigue

These signs are not about “lack of strength” or “motor delay” in themselves.
They refer to a system that does not predict or regulate movement well.
From neuroscience on motor control, we know the vestibular system is a key pillar in the modulation of tone, proximal stability, and postural anticipation, especially during the first years of life, when the nervous system is highly plastic.

Key point:
If the body does not feel secure in space, it cannot organize function or learning.
Thus, the vestibular system is not just a complement to treatment:
It is a structural base for clinical reasoning in neurodevelopment.
