The Diaphragm: The Forgotten Supermuscle in Pediatric Physiotherapy

A Functional and Intersystemic Perspective from TMPI

The diaphragm is much more than a respiratory muscle. In pediatric physiotherapy, this supermuscle holds a central role not only for its part in breathing, but also for its profound interaction with the digestive, circulatory, postural, and emotional systems. At TMPI, we propose a global and intersystemic perspective that allows for its assessment and treatment in a non-invasive, child-appropriate way.


🌧️ Multiple Functions of an Essential Muscle

Respiratory

  • Facilitates alveolar compliance (filling of alveoli like balloons).
  • Improves perfusion and gas exchange.
  • Participates in alveolar recruitment, activating inactive pulmonary zones.

Digestive

  • Its rhythmic movement aids intestinal motility and peristalsis.
  • Regulates the relationship between esophagus and stomach, helping prevent reflux during the first months of life.

Circulatory

  • Supports abdominal venous return and lymphatic circulation, assisting the body’s internal cleansing.

Cavity Separation

  • Divides the thorax and abdomen, and balances with other body diaphragms (pelvic, cranial, plantar, etc.).

Postural

  • Works together with abdominals, lumbar region, and pelvic diaphragm as part of the core.
  • Proper diaphragmatic function promotes the child’s postural development—and vice versa.

Emotional

  • Influences pain perception and emotional regulation.
  • During respiratory crises, effective diaphragmatic breathing reduces anxiety.
  • Hippocrates already described it as “the place where passions and intelligence reside.”

Compromised Circulation

  • Issues with venous and lymphatic return affecting hemodynamic regulation.

Digestive

  • Reflux, hiatal hernias, altered motility.
  • Colic and constipation may have a diaphragmatic origin.

Emotional

  • Anxiety, nervousness, or frequent irritability can be linked to a functional respiratory dysfunction.

Postural

  • Postural alterations linked to the relationship between pelvic and thoracic diaphragm.
  • Imbalance in one causes a chain of compensations.

Cardiac (Functional)

  • Diaphragmatic hypofunction increases cardiac load due to reduced oxygen intake.
  • The heart compensates with faster, shallow breathing.

🏥 Evaluating Without Invading: The TMPI Proposal

Non-Invasive Assessment

  • No need for electromyography or deep palpation.
  • Especially indicated in babies with distended bellies or who show discomfort during internal manipulations.

Intersystemic Approach

  • We consider the interaction between all systems: respiratory, digestive, emotional, circulatory, and postural.
  • We don’t treat only the symptom, but aim to restore functional balance between systems.

  • Our method combines assessment and treatment simultaneously, facilitating changes in diaphragmatic function from the very first contact.

Education in Pure Diaphragmatic Breathing

  • We teach children to breathe from the diaphragm.
  • We especially prepare girls to maintain this breathing before pubertal changes (when they tend to adopt costal patterns).
