In many cases, treatment focuses almost exclusively on the affected muscle and on cervical stretching. The problem is that this approach may prove insufficient if it is not integrated within a global view of the baby.

Understanding CMT only as a muscular problem is one of the clinical mistakes that most affects treatment efficacy. To intervene well, it is not enough to gain range of motion: it is necessary to understand how this alteration influences the baby’s posture, movement, orientation, function, and interaction with their environment.

What Congenital Muscular Torticollis is and why it should not be treated only as a muscular problem

Congenital Muscular Torticollis is an alteration of cervical positioning that is usually associated with lateral head tilt, preferred rotation, and limitation of active and passive movement. Traditionally, much of the intervention has focused on the sternocleidomastoid. However, reducing the whole picture to a single muscle is an oversimplification of the clinical reality.

CMT may have repercussions on the baby’s postural organisation, symmetry, support, visual exploration, and early motor development. For that reason, the treatment of Congenital Muscular Torticollis must be approached from an integral perspective, not a purely mechanical one.

Treatment of Congenital Muscular Torticollis: the importance of a global view of the baby

Effective treatment of Congenital Muscular Torticollis begins with a complete clinical assessment. It is not only about measuring degrees of cervical rotation or detecting tissue tension. It is necessary to observe how the baby moves, how they organise themselves in relation to gravity, how they respond to stimuli, and how they participate in everyday activities.

This global view allows the paediatric physiotherapist to design a more precise, adapted, and functional intervention. Instead of applying techniques in isolation, treatment is built according to specific clinical objectives and the baby’s real progress.

How to design a step-by-step intervention plan in Congenital Muscular Torticollis

One of the most relevant aspects in the management of CMT is planning. Good treatment does not depend on accumulating therapeutic resources, but on knowing how to organise them into a coherent sequence.

Designing a step-by-step intervention plan implies:

identifying the baby’s main limitations

establishing realistic and progressive goals

selecting the most appropriate therapeutic tools

integrating changes into function and into the everyday environment

reviewing the clinical response in order to adapt the treatment

This clinical reasoning is what allows intervention in Congenital Muscular Torticollis to be truly effective and not become a succession of directionless techniques.

The role of the family in the treatment of Congenital Muscular Torticollis

In paediatric physiotherapy, the family should not be understood as a mere observer of treatment. They must form an active part of the therapeutic process.

In CMT, the family is key because they accompany the baby in daily activities, promote opportunities for movement, adapt the environment, and provide continuity to what is worked on in consultation. When the family understands the goal of the intervention and participates in a guided way, the changes are usually more functional and more sustainable.

For that reason, including the family in the treatment of Congenital Muscular Torticollis is not a secondary recommendation: it is a clinical necessity.

Tissue mobilisation, stretching, and manual therapy in Congenital Muscular Torticollis

Within the treatment of CMT, there are different therapeutic tools that may play a useful role when they are applied with sound clinical judgement.

Tissue mobilisation in CMT

Tissue mobilisation may help improve tissue quality and facilitate the work that follows. Used within appropriate clinical reasoning, it may form part of a broader intervention that is better tolerated by the baby.

Cervical mobilisation or stretching in CMT

Cervical stretching remains one of the most commonly used techniques in Congenital Muscular Torticollis. It may be useful, but it should not be applied automatically or become the sole focus of treatment.

PIMT manual therapy in CMT

PIMT manual therapy can be integrated within a global approach, always understanding that no isolated technique can by itself resolve a condition that affects motor development, function, and the baby’s relationship with their environment.

Drawbacks of stretching in Congenital Muscular Torticollis

To talk about the treatment of CMT without critically analysing stretching is to fall short. Although this technique forms part of many approaches, it may also present drawbacks when used without clinical judgement.

Among the most frequent problems are the baby’s poor tolerance, defensive responses, rejection during the intervention, or the decontextualised application of manoeuvres that are not later integrated into function.

For that reason, stretching in Congenital Muscular Torticollis must be considered through clear clinical questions: when to use it, for what purpose, with what intensity, and in combination with which other therapeutic strategies.

The real goal of treatment in CMT: function, development, and participation

In CMT intervention, gaining cervical mobility should not be the only objective. Treatment makes sense when that improvement translates into functional changes: greater symmetry, better orientation, more motor variability, better exploration of the environment, and a more active participation of the baby in daily life.

This approach completely changes the therapeutic logic. The aim is no longer only to correct a specific limitation, but to promote more harmonious and functional development.

Video on the treatment of Congenital Muscular Torticollis

If you would like to explore this clinical approach in greater depth, you can watch the treatment chapter from Iñaki Pastor’s Congenital Muscular Torticollis course on YouTube: treatment chapter on CMT

This video covers manual and neurosensorimotor intervention, tissue mobilisation, cervical mobilisation or stretching, PIMT manual therapy, and guided stimulation with the family, all within a more complete, reasoned, and baby-adapted approach.

Conclusion

The treatment of Congenital Muscular Torticollis should never be reduced to stretching a muscle. That approach may fall short and limit the effectiveness of the intervention.

Current clinical practice requires understanding the baby from a global perspective, designing a progressive therapeutic plan, using the tools with purpose, and making the family an active part of the process. That is the path towards a safer, more functional, and more effective intervention in CMT.

Congenital Muscular Torticollis (CMT) is one of the most common disorders in paediatric clinical practice, yet it is still approached far too often from a limited perspective.
