It’s not just about observing the baby’s mouth, tongue, or lips: the neck and the cervical system play a decisive role in the success of feeding.

This webinar presents a conceptual framework and a practical guide to integrate cervical evaluation into a breastfeeding consultation. A simple, clinical, and applicable approach for health professionals who support families throughout this process.

Why look at the neck in a breastfeeding assessment?

The neck actively participates in three essential functions of the baby:

Feeding

Breathing

Communication

Its role goes far beyond postural support: it orients, stabilizes, protects vital structures, and transmits key sensory information to the central nervous system.
That’s why, when a cervical dysfunction exists — even a minimal one — you may observe:

Difficulty with latch

Ineffective sucking

Irritability

Clear differences between one breast and the other

Postural problems

Alterations in tongue tone or mobility

Integrating neck assessment allows us to detect these difficulties early and guide a more complete intervention plan.

The neck as a system of orientation and stabilization

The baby uses their senses — vision, smell, orofacial touch, and hearing — to orient themselves toward the breast. The neck coordinates these stimuli and facilitates the searching movement and approach to the breast.
For this to occur efficiently, the baby needs:

Adequate mobility

Sensorimotor organization

Postural stability

When these foundations fail, a pattern appears that many professionals recognize: the baby searches but can’t manage to latch; they become frustrated, cry, and cannot maintain a deep latch even when the mother’s position is correct.

Cervical stabilization directly influences control of the jaw, tongue, and lips, and therefore affects sucking, swallowing, and breathing, functions that must coordinate with precision during breastfeeding.

The neck: protector and passageway for systems

The cervical region houses respiratory, digestive, and neurological structures that the baby must coordinate during every feeding. Any postural or functional alteration can modify:

The pharyngeal space

The efficiency of airflow

The quality of swallowing

When an orofacial difficulty is present, the neck often compensates by modifying its physiological curves. And the opposite is also true: a primary cervical alteration can affect the tone and function of the tongue. Understanding this relationship is essential in clinical intervention.

Five steps to integrate cervical evaluation into breastfeeding

During the webinar, we present a simple and structured system, adaptable to different clinical contexts:

1. Targeted anamnesis

Explore prenatal, perinatal, and postnatal history, observe possible asymmetries, ask about maternal discomfort, and gather in detail how the family is experiencing the breastfeeding challenge.

2. Global observation of the baby

Examine posture, symmetry, emotional regulation, mobility, and overall organization. The neck is never alone: arms, hips, face, and skull offer valuable clues.

3. Orientation assessment

Through orofacial touch, observe how the baby directs their mouth toward the stimulus: rotation, sensitivity, motor response, and coordination between the neck and the oral system.

4. Non-nutritive sucking

With the baby stable and calm, analyze sucking in three positions: centered, rotated to the right, and to the left. This test reveals strength, endurance, lip seal, and the quality of tongue movement in each cervical orientation.

5. Clinical evaluation of the feeding session

Using standard tools (LATCH, EPSA, NOMAS, or others), compare both breasts: positioning, latch, sucking, breathing, swallowing, signs of efficiency, and indicators of difficulty.

When several signs align — asymmetries, changes in sucking depending on the side, irritability during feeding, clear preference for one breast — a cervical dysfunction is often present and worth addressing.

The role of physiotherapy in breastfeeding

Physiotherapy, especially from approaches such as Pediatric Integrative Manual Therapy (PIMT), can offer:

Gentle and specific manual treatment

Improvement of cervical mobility

Reorganization of postural control

Re-education of coordination between mouth and neck

Clinical support for the family

When the neck works well, the mouth can also function better. And when the orofacial system improves, the neck organizes itself more efficiently. These are interdependent systems that enhance one another.

Conclusion

The neck is a key player in orientation, stabilization, and sucking efficiency.
Integrating its assessment into breastfeeding consultations:

Improves early detection of difficulties

Allows timely referrals

Strengthens interdisciplinary work

Building networks means supporting families better. And supporting them better means promoting safer, more comfortable, and more successful breastfeeding experiences.

Thank you for joining us in this approach that adds an essential piece to the breastfeeding puzzle.

In the field of pediatric physiotherapy and orofacial care, it is increasingly clear that breastfeeding requires a broader perspective than the one commonly used.
