Neurological tests are within expectations. But something doesn\u2019t fit:  <\/p>&#13;\n\n<ul class=\"wp-block-list\">&#13;\n<li>Why can\u2019t they hold their head steady?<\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li>Why don\u2019t they tolerate prone position?<\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li>Why do they startle so easily?<\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li>Why can\u2019t they organize their movement, even in supportive environments?<\/li>&#13;\n<\/ul>\n&#13;\n<p class=\"wp-block-paragraph\">And then comes the question that can make the difference in your approach:<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\"><strong>Am I taking the vestibular system into account in my evaluation?<\/strong><\/p>&#13;\n&#13;\n<h2 class=\"wp-block-heading\"><strong>A primitive but key system<\/strong><\/h2>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">The vestibular system is<strong> the first functional sensory system in intrauterine life<\/strong>, active from the 8th week of gestation. It is deeply involved in the organization of tone, balance, posture, visual coordination, and emotional regulation. <\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">However, it remains one of the most overlooked systems in the clinical evaluation of babies.<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">How many times do we dismiss the vestibular system because there is no visible injury or an established vestibular diagnosis?<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">How many times do we assume its function is preserved\u2026 simply because the baby does not present spontaneous nystagmus or an obvious balance alteration?<\/p>&#13;\n&#13;\n<h2 class=\"wp-block-heading\"><strong>What the evidence (and the clinic) tells us<\/strong><\/h2>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">Recent studies show that <strong>early signs of vestibular dysfunction in babies<\/strong> may be present even in <strong>the absence of structural lesions<\/strong>. These signs include: <\/p>&#13;\n\n<ul class=\"wp-block-list\">&#13;\n<li><strong>Alterations in axial and cervical tone.<\/strong><\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li><strong>Difficulties in verticalization.<\/strong><\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li><strong>Inadequate postural reactions.<\/strong><\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li><strong>Disorganization of head control.<\/strong><\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li><strong>Hypersensitivity to visual or movement stimuli.<\/strong><\/li>&#13;\n<\/ul>\n&#13;\n<p class=\"wp-block-paragraph\">In a quasi-experimental study on babies with neurological risk, early vestibular stimulation significantly improved head control, visual fixation, and postural tolerance (Trejo-Torres et al., 2019).<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">On the other hand, research such as that of Rine et al. (2024) emphasizes that <strong>early vestibular rehabilitation in pediatrics<\/strong> improves not only balance but also overall coordination, even in populations without an established vestibular diagnosis. <\/p>&#13;\n&#13;\n<h2 class=\"wp-block-heading\"><strong>The risk of not looking<\/strong><\/h2>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">Not integrating vestibular evaluation in babies entails a significant clinical risk:<\/p>&#13;\n\n<ul class=\"wp-block-list\">&#13;\n<li>We may interpret as \u201cslow maturity\u201d what is actually an underlying sensory dysfunction.<\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li>We may delay key interventions during a critical window of neurodevelopment.<\/li>&#13;\n<\/ul>\n\n<ul class=\"wp-block-list\">&#13;\n<li>We may design therapeutic plans without activating the fundamental systems of tonic and postural regulation.<\/li>&#13;\n<\/ul>\n&#13;\n<p class=\"wp-block-paragraph\">And above all: <strong>we may lose the opportunity to offer the baby a more organized, stable, and harmonious development.<\/strong><\/p>&#13;\n&#13;\n<h2 class=\"wp-block-heading\"><strong>How to start looking?<\/strong><\/h2>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">At TMPI, we propose an integrative sensory-motor approach from the very first contact with the baby:<\/p>&#13;\n\n<ol class=\"wp-block-list\">&#13;\n<li><strong>Clinical observation of cervical tone and head support.<\/strong><br\/><br\/><\/li>&#13;\n\n\n&#13;\n<li><strong>Postural and protective reactions in different planes.<\/strong><br\/><br\/><\/li>&#13;\n\n\n&#13;\n<li><strong>Tolerance to changes in position and to gentle vestibular stimuli.<\/strong><br\/><br\/><\/li>&#13;\n\n\n&#13;\n<li><strong>Integration of head, eyes, and body in space.<\/strong><br\/><br\/><\/li>&#13;\n\n\n&#13;\n<li><strong>Coherence between the postural, visual, and auditory profile.<\/strong><br\/><br\/><\/li>&#13;\n<\/ol>\n&#13;\n<h2 class=\"wp-block-heading\"><strong>A paradigm shift<\/strong><\/h2>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">We can no longer wait for the baby to \u201cmature on their own.\u201d<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">The vestibular system is not just a channel for balance.<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">It is a <strong>central organizing system<\/strong> for motor, sensory, and emotional development.<\/p>&#13;\n&#13;\n<p class=\"wp-block-paragraph\">And you, as a physiotherapist, occupational therapist, optometrist, or speech therapist, <strong>can make a difference if you integrate it from the start.<\/strong><\/p>&#13;\n&#13;\n<h3 class=\"wp-block-heading\"><strong>Scientific references:<\/strong><\/h3>&#13;\n\n<ul class=\"wp-block-list\">&#13;\n<li>Bo\u017eani\u0107 Urban\u010di\u010d, V., et al.
