Although they&#8217;re often addressed from a cervical, tension-related, or postural perspective, <strong>eye coordination and oculomotor system function<\/strong> remain largely unexplored\u2026 and are often key in sustaining pain.<\/p>\n\n<p class=\"wp-block-paragraph\">Whether you work in pediatrics or with adults, the oculomotor system should be a fundamental part of both neuromusculoskeletal and neurosensory-motor assessments. That\u2019s why it\u2019s an integral part of clinical reasoning and evaluation in the PIMT Essential training.<\/p>\n\n<h3 class=\"wp-block-heading\">What\u2019s the connection between the eyes and headaches?<\/h3>\n\n<p class=\"wp-block-paragraph\">Many headaches are directly related to the eyes. And diagnostic labels don\u2019t rule out the eyes as a contributing factor. Migraines and tension-type headaches can easily be linked to the visual system\u2014through either oculomotor or visual mechanisms.<\/p>\n\n<p class=\"wp-block-paragraph\">The oculomotor system not only coordinates eye movement\u2014it\u2019s also integrated with postural networks, the vestibular system, and the autonomic nervous system.<strong>Oculomotor dysfunction\u2014such as altered vergence, poor fixation, or slow saccades\u2014can increase visual effort<\/strong>, lead to fatigue, or trigger postural compensations that make the pain chronic.<\/p>\n\n<p class=\"wp-block-paragraph\">Subtle issues in the visual system can also interfere with sensory and postural integration\u2014especially in developing children and school-aged adolescents, where visual and cognitive demands are high.<\/p>\n\n<h2 class=\"wp-block-heading\">\ud83d\udd0d Clinical reasoning: guiding questions<\/h2>\n\n<p class=\"wp-block-paragraph\">These questions can help you decide whether it&#8217;s worth assessing the oculomotor system in a patient with headaches:<\/p>\n\n<ol class=\"wp-block-list\">\n<li>Does the patient report <strong>headaches after reading, screen use, or time in class<\/strong>?<\/li>\n\n\n\n<li>Does the pain improve with eyes closed or visual rest?<\/li>\n\n\n\n<li>Are there compensatory postures while reading (head tilt, very close to the page, closing one eye)?<\/li>\n\n\n\n<li>Do they report <strong>dizziness, instability, or a feeling of visual \u201cdisconnect\u201d<\/strong>?<\/li>\n\n\n\n<li>Is there a history of head trauma, cervical injury, or facial contusions?<\/li>\n\n\n\n<li>Do headaches occur in sensory-stress contexts (bright lights, noise, complex visual input)?<\/li>\n\n\n\n<li>Are headaches accompanied by cervical\/postural discomfort when looking at screens or in the classroom?<\/li>\n\n\n\n<li>Are there difficulties with visual attention or tracking during school tasks?<\/li>\n<\/ol>\n\n<h2 class=\"wp-block-heading\">\ud83e\uddea Oculomotor <em>screening<\/em> tests for patients with headaches<\/h2>\n\n<p class=\"wp-block-paragraph\">These tests can be easily adapted for clinical use at different ages:<\/p>\n\n<h3 class=\"wp-block-heading\">For older children, adolescents, and adults:<\/h3>\n\n<ul class=\"wp-block-list\">\n<li><strong>Smooth Pursuits:<\/strong> Ask the patient to follow a pen or finger in slow, horizontal, vertical, and diagonal motions. Look for jerky eye movements, gaze shifts, or compensatory head movements.<\/li>\n\n\n\n<li><strong>Saccades:<\/strong> Ask them to rapidly look between two points (without moving the head). Assess accuracy and look for corrective movements.<\/li>\n\n\n\n<li><strong>Near Point of Convergence (NPC):<\/strong> Slowly bring an object toward the nose. Note the distance where one eye drifts or double vision appears.<\/li>\n\n\n\n<li><strong>Head-eye dissociation test:<\/strong> Ask the patient to move their head while keeping visual fixation on a still point. Assess visual stability.<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">\ud83c\udfaf What to do if you detect oculomotor dysfunction?<\/h2>\n\n<p class=\"wp-block-paragraph\">If signs of dysfunction appear, treatment options may include:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Performing a complete manual eye evaluation to detect deviations, classify their type, and apply direct manual therapy on the eyes. This often leads to the most surprising results.
